Nominations received for Jindal Prize-2011 and SR Jindal Prize-2011 will also be valid for SR Jindal Prize-2012. For such cases, only the additional information need be sent quoting the JPN Number.

- The application for nomination should be sent in English / Hindi in prescribed format. A self nomination shall not be taken as valid entry.
- All relevant information / material should be attached with the nomination form. No additional information shall be accepted at a later stage except where specifically requested by the Prize Secretariat.
- Attachment / supporting documents will be accepted in English or Hindi so far as they are legible. In case of other languages English or Hindi version certified by a competent authority, may be attached.
- Attachments / supporting documents (photocopies), indicating the nominees scope of work/contribution towards social upliftment may be attached along with the nomination form. Original documents need not be sent. The nominations so received shall be the property of the Prize Secretariat.
- The nomination should be received at the Prize Secretariat on or before the last date as announced by the Secretariat.
